Community Transition Network Hosting Pathways to Transition Event

The Community Transition Network (CTN) is a group that provides the opportunity forThe Community Transition Network (CTN) presents 
Pathways to Transition 

For parents, guardians and students with special needs (ages 14-21) who are in the process of planning for life after high school 

Discover your path with the help of people from 40+ local organizations that provide supports, advocacy, continuing education, employment, funding, recreation, respite, and much more! 

What to Expect at the Event: 
•	Engaging People/Displays  
•	Timely Information  
•	Inspiring Atmosphere  
•	Networking Opportunities 

From 6:30-8:30pm, the Pathways to Transition Event is an opportunity for students, parents, and guardians to freely connect and network with various participating organizations and to gather information and resources from them. Separate and recurring 30-minute sessions will also be offered by: 
•	Developmental Services Ontario  
•	Ontario Disability Support Program  
•	Accessible Transportation Services

community partners in Hamilton to work collectively to support the transition of youth and young adults with disabilities to adult services and living.  

On Tuesday, April 23 from 6:30 to 8:30 p.m., CTN will host a Pathways to Transition event in collaboration with HWDSB. The event will take place at the HWDSB Education Centre (20 Education Court, Hamilton, ON).

The Pathways to Transition event invites students with disabilities and families who are transitioning from youth to adult services to discover community organizations and supports available for students transitioning from secondary school.

Attendees will be provided information on a variety of topics including continuing education, employment, housing, transportation, recreation, and much more. 

30-minute learning sessions will also be offered by local organizations including:

  • Developmental Services Ontario
  • Ontario Disability Support Program
  • Accessible Transportation Services
