Immunization Records and Vaccine Catch-Up Clinic

The Hamilton Wentworth District School Board (HWDSB) and Hamilton Public Health (HPH) are partners in helping young people be healthy, successful in school and positive contributors to their communities.  Students with out-of-date or incomplete immunization records will be suspended from school by HPH.  Records need to be current by March 4th, 2024.

We do not want this to happen.

Please find below information about how to check and update your student’s immunization records.

  • Check your vaccine records at www.Hamilton.ca/vaccines
  • Online Reporting: via URL https://hph.icon.ehealthontario.ca/#!/welcome, or scan the QR code to the right. Please upload a scan or image of the vaccine history document(s).
  • Mail or Fax: Details for mailing or faxing records are provided in the screening letter.
  • Phone Reporting: Call Vaccine Records at 905-540-5250. Expect high call volumes and exercise patience.
  • Records of Exemption also need to be communicated to HPH.

Glendale is hosting a HPH catch-up clinic on February 29th that will allow students to catch up on their required vaccinations. Please reach out to Hamilton Public Health with your questions. 905-540-5250
