We are pleased to offer a vaccine clinic this year at your child’s high school. Students may attend the clinic to receive missing vaccines. The following vaccines will be made available at this clinic:

Tetanus, Diphtheria, Pertussis, and Polio (Tdap-IPV) Tetanus, Diphtheria, and Pertussis (Tdap) Measles, Mumps and Rubella (MMR) Meningitis (Men- C-ACYW135)
Human Papillomavirus (HPV9) Hepatitis B (Hep B) Polio (IPV)

Please visit www.hamilton.ca/highschoolclinics for more information including the scheduled clinic dates. Schools will also remind students about the upcoming vaccine clinic date.

Your child may have received a letter from Hamilton Public Health Services indicating that they have missed a childhood vaccine or a vaccine has not been reported to Public Health. These clinics are an opportunity to update records and/or receive missing vaccinations. It is recommended that your child bring this letter to the clinic day along with their current vaccine record or yellow vaccine card.

Nurses will assess your child’s immunization record on the clinic day and offer any overdue vaccines that are required by the Immunization of School Pupils Act (1991), or that may have been missed in grade 7. Prior to receiving vaccines, students must provide informed consent at the school-based clinic prior to vaccination as per the Consent to Treatment Act (1992).

If you choose to have your child vaccinated by another health care provider, please remember to update your vaccine record with Hamilton Public Health. You can also view your child’s vaccine record, or report vaccines to Hamilton Public Health by visiting hph.icon.ehealthontario.ca, or by calling 905-540-5250. Please note that family health care providers do not report vaccines to Hamilton Public Health.

If you do not wish your child to be vaccinated at school, please instruct your child not to attend the clinic. Students will not be called down to the clinic, they will be able to come to the clinic during designated times through out the school day. Only students who attend the clinic will be assessed and offered outstanding vaccines.

On the day of the clinic, please ask your child to wear a short sleeve t-shirt.
