
Medical Plans of Care
Dear Parents and Guardians:
The health and safety of our students is a priority at Norwood Park School, and we would like to remind all parents/guardians that students with life threatening medical conditions require a plan of care. Some students may have a prevalent medical condition while others have medical conditions that require a medical response.
Prevalent Medical Conditions
Asthma
Diabetes
Epilepsy
Anaphylaxis
If your child has one or more of the listed prevalent medical conditions, please contact the main office. A plan of care will be put in place to support the health and safety of your child.
Non-Prevalent Medical Conditions
In some cases, students may require a plan of care to be put into place for conditions that are not one of the above listed prevalent conditions. Please contact the main office for support on determining if a plan of care needs to be implemented.
If your child has one or more of the listed prevalent medical conditions, is experiencing similar symptoms, and/or is being tested for a life-threatening condition, please contact the main office immediately. An emergency response plan of care will be put in place to support the health and safety of your child.
