Permission Forms to Access Community Centre Fields

Hello Parents and Guardians,

Now that we are in our new location, we are in the unique position to be able to make use of the fields surrounding the Beverly Community Centre directly behind the school.  We have an agreement with the City of Hamilton to be able to make us of these fields, and we would like to have your consent to allow your children to have access to the fields when accompanied by a teacher.  Students would not be able to use these fields for nutrition breaks, but they would be allowed to take walking trips with their teachers for the purposes of an outdoor education or physical education activity.

Please print out the attached forms (3 pages) and return them to your child’s homeroom teacher as soon as you are able.  If you would prefer to receive a paper copy from the school, please send a request with your child, or email the homeroom teacher.  Thanks for your support!
