Vision Works Clinic Permission Form Distribution

Parents & Guardians,

Please note that Parents/Guardians should receive hard copies of a permission form for an upcoming, “Vision Works Clinic” which will be taking place at Highview School on April 9th, 10th, and 11th. This is an opportunity to have an eye examination done on your child by professionals right at the school. Please note that this initiative is for children who are NOT CURRENTLY SEEING AN OPTOMETRIST ON A YEARLY BASIS and especially children who have NEVER HAD THEIR EYES EXAMINED by an Optometrist. Please see the permission form for more information. Please note that along with the eye exams, for those who are in need of glasses, glasses will be made available for the subsidized cost of $20 or a prescription provided.
