Student Verification Forms

In the next few days, your child will bring home the Student Verification Form which will need to be reviewed by you.

It is important that the school has the most up-to-date information in the event of an emergency and need to reach you.  If anything on the form is incorrect (or additions need to be made), please make the necessary changes right on the sheet (preferably in red).  If any changes are being made to the Immigration/Miscellaneous portion of the form, documentation must be presented to the office.  This information will be photocopied and kept in your child’s file.

Please have your son or daughter return these forms (even if there are no changes) to your child’s teacher by October 18, 2019.
