New Kindergarten Registration – Vaccines needed for School Entry

Dear Parent/Guardian,

Every year, Hamilton Public Health Services reviews the vaccination record of every child attending a school in the Hamilton area, to ensure their vaccinations are up to date according to Ontario’s Immunization Schedule.

For new students entering the school system this fall, Hamilton Public Health would like to make parents/guardians aware of their responsibility and the process to inform HPH of the vaccine status of our new kindergarten students.  The attached flyers explain the requirements of Hamilton Public Health, how to submit the information, and the necessary forms to submit the information.

Please review the forms and information sheet and submit your information to Hamilton Public Health.  The school is not involved in the information gathering or retention of vaccination information.
