GRADE 7/8 SCHOOL VACCINATION CONSENT FORM

If you haven’t already ready returned your school vaccination consent forms please send them in ASAP.  The consent forms were due in the office on October 23, 2018.  The vaccinations take place on November 9th.  Thank you.
