It is the Board’s policy that all students who require a medical plan of care are clearly identified. For your convenience, we have attached forms for the following prevalent medical conditions:
If applicable, please copy and complete the appropriate form for your child in consultation with your family physician over the summer holiday. Providing families with this information now will alleviate some of the stress that comes with September Start-Up.
We have also attached 2 other forms for completion should your child require medication administered at school. Please choose the form that best meets your child’s needs for the 2021/2022 school year.
