Before and After School Care

“Thank you for taking the time to complete the Before and After-School survey for your child(ren) in Kindergarten to Grade 6.   A child care agency has yet to be assigned to provide a before and after school program at Lake Avenue School beginning September 2018.  We are in the process of conducting a “Request for Proposal’ process to determine who will be the provider.  Once a provider is assigned, we will inform you. If you completed the Before and After-School survey earlier this year, and you provided consent to release your information, we will be providing that information to that agency so that they can contact you to complete your child’s registration.
