IMPORTANT: Medical Information & Media Consent

Hello Billy Green Families!

An important part of the information exchanged between school and home is medical information AND media consent.  Medical contact information has in the past been gathered through forms that you would receive for every excursion, sports event or any off-school property event.  At times, in the course of a year, you may have filled out the same form 2, 3, or 4 times or more.  As part of our caregiver/parent survey, we are looking to streamline this process.

What to do next? Please go into the parent portal and please update your child’s current medical information including health card information.  Also, please be sure to click on whether you provide for media consent by clicking YES or NO.

Using the parent portal to update this information year to year will eliminate the need for the school to send out paper copies for each off-site event.  This means that you will not need to fill in the medical form package multiple times during the year.  It will also allow you to update information as needed.

If you need support in this process, please reach out to the school office.  Mrs. Hyatt or Mrs. Barker will be more than happy to help.  We ask that you update the forms in the portal no later than October 1, 2025.
