Please note that there are several updated forms available in the Parent Portal:

• Student Information Verification form
• Early Closure and Emergency Evacuation Information form

Please complete these at your earliest convenience so that we have the most accurate and up to date information.

Also available is:

• The Attestation for Return to School Following Illness or Self-Isolation form
  

This form is used by families to document that it is safe for your child to return to school or child care after an illness or self-isolation time.
