Updating Forms for Students with Anaphylaxis and Other Health Concerns
In an effort to keep our school and classrooms safe for all of our students, we are asking that the parents of Anaphylatic students and/or those with severe Asthma, update the specific Emergency Procedure Forms with with the most current information they have. The school will be providing updated pictures for the updated forms.
Should your child have any other medical concerns, please ensure that you have included them on the Emergency Health Form that was sent home during the first week of school.
Copies of the Anaphylatic, Asthma, and Emergency Health Forms are attached to this post, and are also available for pick-up at the main office.
We would ask that all of this information be completed/updated by Friday October 6.
