Student Anaphylactic/Medical Forms

Please drop your child’s epi-pen, anaphylactic form (pink), asthma management form (yellow) and/or prescription medication forms (blue) to the office this week (Office Hours 8:00 am to 2:00 pm).

If you are unavailable to come in, please ensure the forms and epi-pens are in your child’s backpack first day of school.
