REVISED: Emergency Student Medical Information Form

Hello Westwood,

On Thursday, October 5th, at the end of the day, the school will be sending home a form titled, “Emergency Student Medical Information Form”.

We kindly ask you to review the contact information sections carefully so that the school has your updated contact information in the event of an emergency.  It is also important that the school has updated information on who can pick up your child from school in the event of an emergency.

We ask that this form is returned no later than Tuesday, October 10th, even if there are no errors.  Please sign and initial that you have reviewed the form and all information is correct.  If there are any changes, please make them directly on the form.
