CanChild Research Study of Occupational Therapy Services-Survey of Families

 

 

Creating a Road Map for Successful Implementation and Scale-Up of Partnering for Change (P4C) Tiered Services in Ontario: A Phased, Prospective Realist Evaluation

 

Our school is taking part in a research study being done by researchers from CanChild Centre for Childhood Disability Research at McMaster University. The researchers are doing this study to learn more about the occupational therapy services provided in our school.

 

This email is being sent on behalf of the researchers at McMaster to invite you to take part in their research study. The researchers do not know who has received this invitation. It will not be possible for the researchers to know your identity unless you choose to respond.

 

Invitation to Participate in Research Study

 

You are invited to complete this survey to help us learn important information about the occupational therapy service underway at your child’s school. As part of this service, an occupational therapist is working with teachers in classrooms and during school activities. They also may share information with families that can help support children at home.

 

We would like to know more about families’ awareness of the services being provided, and their experiences and satisfaction with any services received. All families are invited to participate.

This survey will take no more than 10 minutes to complete.

 

It is expected that there will be no risks to you in taking part in this confidential survey.  You can stop at any time. You also may choose to exit the survey at any time. If you exit the survey before submitting your answers, your responses will not be included in our study.

 

If you choose to complete a survey, you will be able to share your contact information to receive a $10 e-gift card.
