Concussion Forms
Due to new policy with regard to concussions, paperwork is now necessary to complete at school and return to the parent. As a result, if you received this yellow / orange form (“Tool To Identify a Suspected Concussion”), it is imperative to fill it out and RETURN it back to the school as soon as possible. There are a number of forms outstanding, and we would greatly appreciate your assistance in returning them at your earliest convenience.
