Vaccination Clinic at Collegiate Elementary School

Please be advised that Collegiate Elementary School will be hosting a school vaccine clinic, Round #2 of Hepatitis B, Meningitis, and HPV vaccines for students born in 2009 (and 2010).  If you have already submitted a consent form to the school, no further action is required.  If, however, you would like your child to receive these vaccines and have not already signed the form, please complete a form that will be supplied at the school office.

Public Health will be coming to Collegiate Elementary School on Friday, June 9th.
