Grade 7 & 8 Round 2 Vaccinations

English Fact Sheet-With HPV- 2023

This email is to inform families that Hamilton Public Health will be coming back to Chedoke Elementary School on Monday April 22nd, 2024, to complete routine vaccinations for Grade 7/8 students (Meningitis, Hepatitis B, and HPV). Only grade 7 students who returned a consent form during round 1 in fall 2023 will be vaccinated if they are due for another dose. Any grade 8 students who returned a consent form last school year and remain incomplete will be immunized as well. You do not need to submit another form if you have already done so in fall 2023.

If your child did not submit a vaccine consent form or is new to the school; but would like to receive the vaccines that are offered in Grade 7/8 (Meningitis, Hepatitis B, HPV), you may complete a vaccine consent form and return it to the school. Electronic copies of the consent form has been provided to the school. · Walk-in clinic, OR · Hamilton Public Health Community Clinic – book an appointment. Bookings open as clinic dates are confirmed. More clinic dates will become available throughout the school year and into the summer.

· You do not need to bring a consent form with you to these clinics.

Report Your Vaccines to Public Health

· Oftentimes, family doctor’s offices, walk-in clinics, and hospitals do not report immunization records to Hamilton Public Health.
