Public Health Grade 7 Immunizations
November 6, 2023 - November 7, 2023
Permission forms from Public Health will be sent home for grade 7 students the week of October 10, 2023.
Grade 7 students with signed permission forms will receive the following vaccinations:
- 1st dose of hepatitis B
- 1st dose of HPV
- One dose of meningococcal
Any questions or concerns should be directed to the Vaccine Program for the City of Hamilton Public Health Dept at 905-546-2424 ext. 7556.
Please note the school does not maintain vaccination records; only your family doctor and the City’s Public Health Dept have that information.
