Immunizations Part 2

This is a continuation of the immunizations received from Public Health in November.

The Vaccine Program will be here all day on May 7th, and the morning only on May 8th.  If you have any questions or concerns about immunizations, please call the City of Hamilton Public Health Department Vaccine Program at 905-546-2424 x7556.

The school is unable to answer questions regarding your child’s immunization records.

Only students who returned the signed consent forms in November will be vaccinated…no other forms will be sent home.
