Lincoln Alexander School – Form Submission Reminder

Parents and Guardians,

Please be reminded that we are asking that the following forms be submitted through the “HWDSB Parent Portal” for each child at Lincoln Alexander School if you have yet to do so. Thanks to those who have already submitted these.:

• Digital Tools Permission Form/Annual Media Consent Form (Found under Forms > Permissions > Permissions & Agreements within the Parent Portal)
• Student Information Verification (Found under Forms > Registration > Student Information Verification)

Thanks in advance for your attention to this as it is very important for us to have this information on file, especially current contact information in the event that the school needs to contact you regarding an emergency situation involving your child.
