Student Screening – Attestation & Daily Tracker
Central Families,
Before your child’s first day of school, please complete the Attestation form in the Parent Portal.

On their first day all students will be given a paper copy of our tracking calendar CENTRAL Daily COVID-19 Screening Verification Log (1)
Each morning, parents/guardians are to:
(1) Complete the Covid-19 School and Child Care Screening

(2) Initial the paper tracking calendar to indicate that your child has “passed” the screen and may come to school
Students will need to show this tracking calendar to their Period 1 teacher each morning.
Students who are able to independently read all the information and complete their own screening on the Ministry site (https://covid-19.ontario.ca/school-screening/), may do that independently and show their period 1 teacher the green “passed” screen.
