Green Venture Waiver Form

Please find attached the waiver form for our planting day scheduled for October 28th.  All participants must sign a waiver.  Waiver forms will be available at the event.

If students wish to volunteer (and we really want them to!) and they aren’t coming with their parents, then parental permission is required.  Please print and sign off and being to the event on Saturday, October 28th.  The event begins at 9:00am and we will continue to plant until we are done!
