School forms
Hello Spring Valley Parents and Caregivers:
Each September we send home a package containing a variety of forms (internet use, media consent, student verification, etc.). This year, due to our Covid 19 protocols, we are asking you to complete and submit the forms on line. The following forms have already been uploaded to the Parent Portal.
•Digital Tools Permission
•InternetUse and PhotoConsent
•Media ConsentAgreement
•Elementary: Nutrition Break Consent
These forms will be available later in September:
•Student Information Verification
•Elementary: Emergency School Closure
In addition, the following forms continue to be available on the Parent Portal for parents to complete.
•Student Screening Declaration (Attestation)
•Student Mask AttestationException
To access these forms, parents can log in to the Parent Portal and then click on “Forms”on left hand side. You will then see a screen with all the forms available to choose from. Forms will be added in the following weeks and we will let you know as new ones become available.
