Return to School Attestation
Before starting school in person, a student’s parent/guardian/caregiver or the student over age 18 must complete a one-time attestation regarding daily screening for COVID-19 symptoms.

Before returning to school after an illness or failed screening, a student’s parent/guardian/caregiver or the student over age 18 must complete an attestation to confirm they are eligible to return to school.

Fully Vaccinated Attestation for Return to School
Close contact students of a positive COVID-19 case who are fully vaccinated may be eligible to return to school after a class is dismissed. A student’s parent/guardian/caregiver or the student over age 18 must complete an attestation to confirm they are fully vaccinated.
