Millgrove Agenda Pages – Revised

Attached is a revised copy of the Millgrove agenda pages.  Please note the documents now available in the parent portal for completion.  Please note that we expect all students to stay at Millgrove for lunch so the Nutrition Break Program form can remain empty for the moment.

*ANNUAL MEDIA, PHOTO USE, AND INTERNET USE CONSENT AGREEMENT– in the Parent Portal

HWDSB has a “Media Consent Agreement” for using the internet. The signatures of the student and parent/guardian on the form are mandatory before access is granted for use of the internet on the Board’s system.   There is also an “Internet Use and Photo Consent Agreement”.  Both must be submitted online this year.
