Informed Consent, Privacy and Data Security Statement for the Parent Voice Survey

This page describes how we collect and handle the information from the Parent Voice Survey.

At Hamilton-Wentworth District School Board (HWDSB), the protection of your personal information is a top priority. Our Research and Analytics Department receives and securely stores all surveys. When parents complete a survey online, their responses are kept on a secure server.  Paper surveys are entered electronically by the Research and Analytics Department staff and are stored for up to one year in a locked filing cabinet, in a locked room and then securely shredded.

All survey data is accessed only by researchers in the Research and Analytics Department for the purposes of analyzing, summarizing, and writing reports on the findings. The data analysis involves grouping similar opinions to reveal themes for each question so that we can learn overall, what parents think about our progress in each of the areas we ask you about. We summarize results at a school and board level and communicate the results back to the HWDSB community.

Information you submit to HWDSB is protected under the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA). That means that the information we collect through this survey cannot be used or shared for any purposes other than the original purpose that we described.
