Community Transition Network Hosting Pathways to Transition Event
The Community Transition Network (CTN) is a group that provides the opportunity for community partners in Hamilton to work collectively to support the transition of youth and young adults with disabilities to adult services and living.
On Tuesday, May 2, from 6:30 to 8 p.m., CTN will host a Pathways to Transition event in collaboration with HWDSB. The event will take place at the Nicholas Mancini Center (44 Hunt Street, Hamilton).
The Pathways to Transition event invites students with disabilities and families who are transitioning from youth to adult services to discover community organizations and supports available for students transitioning from secondary school.
Attendees will be provided information on a variety of topics including continuing education, employment, housing, transportation, and much more.
