If you cannot complete the attestation(s) through the Parent Portal: email a signed copy of the form to the school; or, complete the form and call your school to arrange a drop off time when staff can receive the form through a contactless process. Daily COVID-19 Screening
Review this screening for COVID-19 symptoms daily before your child arrives at school. If you answer Yes to any questions, the student has failed the screening and must not attend school and should contact Public Health (905) 974-9848 for testing.
Return to School Attestation – after illness or self-isolation
This is an attestation for return to school/child care following illness or self-isolation. Parents and families, please complete this form to document that it is safe for your child to return to child care or school (elementary and secondary).
Return to School Attestation – before in-person learning
Before starting school in person, a student’s parent/guardian/caregiver or the student over age 18 must complete a one-time attestation regarding daily screening for COVID-19 symptoms.
Student Mask Exception Attestation
Complete this form if your child/adult student claims an exception to the requirement to wear a mask while in a school or school bus.
Readiness for Distance Learning
In the event that we have another school closure because of COVID, we are asking families to assess their preparedness to have students learn from home. Please fill out one form for each of your children. Families can complete the form and share with their school. This form can also be found on the Parent Portal.
