Daily Screening, Forms and Translations

These important forms are available for parents/guardians (or students over age 18) to complete on the Parent Portal, which can be translated into French, Spanish, Arabic, Urdu, Punjabi, Chinese, and Serbian. If you cannot complete the attestation(s) through the Parent Portal: email a signed copy of the form to the school; or, complete the form and call your school to arrange a drop off time when staff can receive the form through a contactless process.

Please note: All paper forms should be submitted directly to your child’s school.

Daily COVID-19 Screening

Daily screening for COVID-19 symptoms and staying home if you are sick remain important ways to reduce the spread of the virus. Please complete this daily for each child using the updated provincial screening, which is updated with symptoms typically seen with Omicron and new guidance on testing and isolation periods.

All schools must also return to the use of daily confirmation that a student in Kindergarten to Grade 12 has completed the screening. Confirmation can be done by showing an educator the green check mark at the end of the screening, that confirms a pass; or they can present a family signature on the monthly attestation calendar available online.

Before starting school in person, a student’s parent/guardian/caregiver or the student over age 18 must complete a one-time attestation regarding daily screening for COVID-19 symptoms.
