Transcripts

Former graduates and retirees can obtain a copy of their transcript of marks. You will need to provide identification such as a driver’s license, health card, or birth certificate when making the request. Please allow up to a week for processing, depending on demand.

Note: If you completed high school within the past year, please contact your school directly for a copy of your transcript.

Diplomas

Graduates can obtain a copy of their High School Diploma by first contacting their school directly.

High school Diplomas can also be obtained through the Assessment Centre by completing the School Diploma Request form. A non-refundable fee of $30.00 must accompany the request.

Recent Graduates/Retirees

If you have completed high school within the past year, please contact your high school for a copy of your transcript. Marks are not filed centrally until one year after you leave the system. For school contact information, please view our Schools Listings.

Former Graduates/Retirees

You have the option of ordering your transcript in person or downloading the form and mailing it in to request your transcript.

In Person

The Student Records Department is located in the Assessment Centre at the Education Centre, 100 Main Street West, Hamilton. Ontario L8P 1H6 (Corner of Main & Bay Street - Map).

  • Apply in person at the Assessment Centre on the first floor and complete the School Transcript Request Authorization and Consent Form.
  • Office Hours: Monday to Friday, 8:30am - 4:00pm. Holiday and summer hours 8:30am - 3:00pm.
  • There is a $20 non-refundable fee for a student transcript that must be paid prior to processing. There is also a $5 fee for each additional copy if requested at the same time.
  • Payment can be made with cash, cheque or money order, payable to The Hamilton-Wentworth District School Board. A receipt will be provided.
  • Transcripts can be forwarded by mail or can be picked up.

By Mail

  • Please print and complete the School Transcript Request Form
  • There is a $20 non-refundable fee for a student transcript that must be paid prior to processing, there is also a $5 fee for each additional copy if requested at the same time
  • Payment can be made with cash, cheque or money order, payable to The Hamilton-Wentworth District School Board. A receipt will be provided.
  • Mail the completed form and payment to:

** The Transcript Request Form cannot be sent via e-mail, as a signature and payment are required.

If you live outside the Hamilton area and cannot come in to the Education Centre to pick up your transcripts, a faxed request would be accepted if payment can be made at the Board Office through a relative or a friend in the Hamilton area.
